Adjunct New Hire/ Rehire Paperwork
Please complete and return your paperwork to the Office of Institutional Research, Dwight Hall, Room 113. If you have any questions regarding your paperwork, please contact Erin Reardon at 508-626-4115.
The following documents must be printed, completed and returned:
- Demographic Data Form
- W-4
- M-4
- I-9 Employment Eligibility Form (*Must be completed/verified within first 3 days of employment.)
- Direct Deposit
- OBRA
- HIRD Form
- SSA-1945 Statement Concerning Your Employment in a Job Not Covered by Social Security
- State Ethics Requirements Memo (*Must complete Ethics Test and send certificate to Human Resources within the first 30 days of employment)
- Payroll Public Exemption Form (optional)
- Voluntary Self-Identification Form - Race & Ethnicity (optional)
- Voluntary Self-Identification Form - Veteran/ Disability Status (optional)

You must read, understand, and acknowledge the following law/policies:
- Introduction to the Conflict of Interest Law for Public Sector Employees (Must be acknowledged annually)
- Policy Prohibiting Sexual Harassment, Sexual Assault & Sexual Violence (Must be acknowledged annually)
- State Ethics Requirements Memo (Must be acknowledged every two years)
