other charges for day school
students.

This publication contains day
school student information on
procedures for paying the Student Bill; tuition,
fee, room, board and other
charges; refund policies;
health insurance requirements;
and special reminders.
Monthly Payment Option through Tuition
Management Systems
The "Monthly Payment Option"
offered through Tuition
Management Systems enables day
school students to budget all
or part of the Student Bill
without interest. The Plan may
be used in conjunction with
all forms of financial aid
such as grants, loans, and
scholarships. Monthly payments
must be made to TMS by the
first of each month.
You may participate in the
Plan by budgeting costs from
June 1 to October 1 for the
fall semester, and from
November 1 to March 1 for the
spring semester. You may also
budget for a single semester.
You must enroll and make your
first payment prior to the due
date of the Student Bill.
In addition, you must make the
appropriate adjustment on the
reverse side of your Student Bill, sign the
certification on the front and
return the top portion to the
student accounts Office by the
due date of the Bill. This
will inform the College of
your enrollment in the
"Monthly Payment Option" and
the budgeted amount.
The "Monthly Payment Option"
is available for a
small enrollment fee and
includes personal account
service, automated account
information 24 hours a day,
access to your account through
their web site,
www.afford.com,
and even gives you a chance to
win a Tuition Management
Systems Scholarships.

Health Insurance Requirement
To our
students and their
parents/guardians:
State
Law
requires
all
full-time
students attending
Framingham State College to
have Health Insurance.
In
order to comply with this
law and at the same time
promote good health among
students, Framingham State
College joined with other
State Colleges in making
available to qualifying
students a comprehensive and
affordable student health
insurance plan.
Only full-time students are
eligible to enroll in the
plan.
If you
are a
full-time
student, whether in the Day
College, Continuing
Education or a combination
of
both,
you
MUST
choose one of the following
two options:
1.
You may
participate in another
insurance plan
IF
you determine that plan to
be comparable or superior to
the plan outlined in the
brochure available on-line.
If this option is chosen,
you
must
complete on-line a “Waiver
Application”.
Once
you are on the site, you
will be asked for the
following information about
your coverage:

Name and address of
insurance company,
policy number and name
of subscriber
-
Policy limits (lifetime
maximum should be at
least $50,000)
-
Will it cover you in the
Framingham area for both
in-patient and
out-patient care
including mental health
-
Will it remain in force
through the 2007-2008
academic year
-
Does it provide coverage
for pre-existing
conditions
Please
note that coverage by an
insurance company outside
the United States or
coverage under a National
Health Service Program from
another country is not
acceptable for a waiver.
OR
2.
If you do not have health
insurance coverage, you must
enroll in the health
insurance offered by the
College.
In
order to enroll, print out
the “Health
Insurance Application Form”,
complete and submit the
Application and payment of
$1,251.00 by the due date of
the Student Bill to the Student Accounts Office.
Payment may be made by
check, MasterCard or Visa.
The Application cannot be
completed on-line. Please
note that financial aid
funds and the payment plan
may not be used for payment
of the Student Health
Insurance Plan premium.
The due
date for completing the
“Waiver Form” on-line or
submitting the Insurance
Application and Payment is
the due date of the
Student Bill.
A
Waiver or Application must
be done on a yearly basis
for all full-time students.
Part-time students are not
eligible to purchase the
health insurance offered by
the College
FAILURE
TO CHOOSE EITHER OPTION 1 OR
OPTION 2 BY THE DUE DATE
WILL MEAN THAT YOU ARE NOT
ENROLLED AT THE COLLEGE AND
ANY COURSES FOR WHICH YOU
HAVE REGISTERED WILL BE
FORFEITED AND YOUR RESIDENCE
HALL ROOM RESERVATION AND
PARKING ASSIGNMENT CANCELED.
Additional health insurance
is available for family
members and may be selected
on-line.
These plans are voluntary;
however,
THE
STUDENT
MUST BE
INSURED.
The
student health insurance
described in the brochure
will supplement the
comprehensive services which
the College Health Center
will provide to those
students who have completed
the Health forms and paid
the mandatory College Health
Service Fee. The 2007-2008
Health Insurance brochure is
available on-line through
the College website at
www.framingham.edu,
selecting Health Services
under the Quick Links drop
down box, selecting the
Health Insurance tab,
clicking on the line “To
Waive the Insurance” and
then selecting the Benefit
Details tab.
