How to Obtain the Discount

Individuals interested in taking advantage of this corporate discount should fill out this form and then have it signed by your Human Resource Representative then provide it to the Office of Student Accounts, located in the Student Service Center, 5th Floor, McCarthy Center. This process must be followed every semester enrolled into a course if individuals want to continue with the discount program.
