Admission Health Forms and Medical Requirements
All forms are to be submitted online via the Medicat Student Health Portal:
- Graduate Students: Full time (2 or more classes)
- Undergraduate students: full time (3 or more classes)
- Health Science Majors: full time and part time
- International students attending on a student visa: full time, part time and visiting
- All students applying to live on campus
The Commonwealth of Massachusetts requires that all full-time students (any day/night combination of three or more courses) participate in a health insurance plan. To fulfill this requirement, you may either:
• Enroll in the Massachusetts State System Plan, which was designed for state college students or
• Show evidence of a comparable or better health insurance plan of your choice.
You will receive information with your bill in July about enrolling in the school-sponsored health insurance plan or waiving it with your own insurance. To enroll in the school-sponsored health insurance plan or to waive the school plan with your own health insurance plan, click here.

In accordance with the Dean of Students Office’s Absence Notification Policy (http://ramhandbook.com/absence-policy-2/) the Health Center does not provide notes excusing students from class or other academic obligations for brief absences due to illness or injury. We advise students who miss class to communicate directly with their faculty/instructors in a timely manner. Students are encouraged to contact the Dean of Students Office for extended absence support.
