New Faculty, Staff, and Contractor ID Cards

New Faculty, Staff, and Contractors should fill out a New Employee ID Application. This application will need to be signed by their employer and include the necessary access for the employee. This form should be turned into the ID Office, located in the McCarthy Center (ground floor) next to the University Police. The new employee will need to have a valid ID, such as a Driver's License or Passport, for verification. The new employee will need to know their FSU ID Number. The new employee will not receive their FSU ID without a valid ID.

ID cards must be turned into the ID Office if the University employee transfers to another department or is no longer employed by the University. The employees' final paycheck may be held pending return or clearance of outstanding IDs.

ID cards needed by outside contractors must be authorized by the Facilities Department, including individual name, firm name, and specific dates the contractor will be on campus.

New Student ID Cards

Most new students will obtain their ID cards during Orientation's or “Taking Care of Business” sessions. If the new student has not obtained their ID card during these sessions, they can come to the ID Office, located in the McCarthy Center (ground floor) next to the University Police. The student will need to fill out the New Student ID Application and have a valid ID, such as a Driver's License or Passport, for verification. They will also need to know their FSU Student ID Number. The new student will not receive their FSU ID without a valid ID.

Lost ID Cards

If a student loses their ID card, they will need to come down to the ID Office, or the University Police to report the lost card immediately. The student must then fill out a Replacement ID Card Form, located in the ID Office. With the completed form, they will have to pay a 20-dollar fee in the Student Accounts office, located on the 5th floor of the McCarthy Center. Once the payment is completed, the student can then bring the form back to the ID Office to obtain a replacement ID card.

If a Faculty, Staff, or Contractor loses their ID card, they will need to come down to the ID Office or the University Police to report the lost card immediately. The employee can obtain a replacement ID card from the ID Office.

Stolen ID Cards

In the case of theft, the individual will be responsible for providing the ID Office a copy of the police report from the respective department. The student will not be charged for a replacement ID if a copy of the police report is provided to the ID Office.

Temporary ID

Temporary IDs may be given to students who have lost their ID card. This can be obtained at the ID Office or the University Police (4 PM- 8 AM). Each student can receive ONE Temporary ID per lost ID card. A Temporary ID lasts 24 hours, unless obtained on Friday after 4 PM, in which those cards will last until 8 AM on the following business day. Once you receive your Temporary ID, you will need to go to Dining Services to access the Dining Hall. To enter your assigned residence building, you will have to call the University Police to be let in. If you do not have a Temporary ID, you will not be admitted into your residence building. Once you are admitted, you will then need to show the SDA your Temporary ID for verification purposes.

Access and Clearance Requests

All access and clearance requests must be submitted to idoffice [at] framingham.edu. Access to some rooms or buildings may need to be approved by other departments before the clearance is added. All resident hall access MUST be approved by ResLife.

The ID Office is CLOSED on Holiday's and during School Closures.
